Description'Dear Sir,
Enclosed I send proof of a blank form which I have devised, & which I propose to send out immediately to all those from whom a Report will be due on Jan [January] 31st. The object is to get the Reports in more regularly and in more uniform style. I intend to send it out at the beginning of every year. If you approve of the form kindly return it to me for press. I enclose Mr Glazebrook's proof, which he says is to be submitted to you.
