Description'Dear Sir,
I am instructed to ask if you would be so good as to fill in the accompanying form and return it to the Secretaries. A form of the kind was, I believe, filled in at the time of your election, or should have been, but it is unfortunately missing from its place among our documents, and as the information is now required for making certain averages, the Secretaries trust that you will comply with this request.
